二、申请人的资格要求：

normal style='text-indent:28.0pt;line-height:150%'>（2）具有良好的商业信誉和健全的财务会计制度：提供2020年度财务审计报告（包含资产负债表、利润表(或利润表及利润分配表)、现金流量表和财务报表附注），审计报告未加盖审计机构公章视为无效；或提供银行2020年至今出具的有效资信证明；或承诺具有良好的商业信誉和健全的财务会计制度；

normal style='text-indent:28.0pt;line-height:150%'>（3）具有履行合同所必须的设备和专业技术能力：提供具有履行合同所必需的设备清单和专业技术能力人员名单；或承诺具有履行合同所必需的设备和专业技术能力；

normal style='text-indent:28.0pt;line-height:150%'>（4）具有依法缴纳税收和社会保障资金的良好记录：提供2021年1月至投标截止时间任意一个月依法缴纳税收的凭证（未发生缴税情况的，须提供零申报证明，即提供企业自行在网上申报系统中打印的已申报报表；依法免税的，须提供企业所在地税务部门出具的相应证明）；或承诺具有依法缴纳税收和社会保障资金的良好记录；
